If you plan on going through your insurance to cover your treatment cost, please submit the following information BEFORE you book your first visit. We will inquire directly about your coverage and benefits, including: co-pay, number of visits allowed, plus any deductible you may be subject to. However, this verification is only an estimate based on what your provider reports to us. If your insurance denies payment of a claim or requests a re-payment for any reason, you are responsible for the billed charges. Please note that if you book an appointment without first having your coverage verified, we will collect our usual service fees at the time of your appointment. Thank you.
